About This Role
We are seeking dedicated Occupational Therapists to deliver one-on-one home health Boca Raton, Delray, Boynton Beach and West Palm Beach. In this role, you will help patients rebuild their daily living skills, improve safety, and restore independence through personalized, goal-driven treatment plans. You will evaluate patients’ functional abilities, develop individualized care plans, and provide hands-on therapy to improve independence in daily living activities. You will also assess home environments for safety, recommend adaptive equipment, and collaborate closely with physicians and interdisciplinary care teams to ensure coordinated, high-quality patient care.
Key Responsibilities
- •Evaluate patients’ strength, coordination, endurance, and self-care abilities
- •Assess home environments and recommend safety modifications or adaptive equipment
- •Develop and implement individualized treatment plans with measurable goals
- •Provide therapeutic interventions to improve daily living skills
- •Fabricate splints and train patients in adaptive, prosthetic, and orthotic use
- •Communicate patient progress with physicians and care teams
- •Maintain accurate and timely clinical documentation
- •Educate patients, families, and caregivers on care plans and progress
- •Participate in care coordination, case conferences and discharge planning
