About This Role
Our CarePine Stroudsburg office is seeking passionate and dedicated Licensed Practical Nurses (LPNs) to join our Skilled Nursing team. The ideal LPN must have pediatric experience providing personalized patient care in home and school settings.
Key Responsibilities
- •Provide one-on-one skilled nursing care in the home and or school setting
- •Deliver treatments and nursing services as directed by the care plan
- •Monitor patient conditions and report changes to the supervising Registered Nurse
- •Support implementation of individualized patient care plans
- •Educate patients and families regarding ongoing care needs and treatment plans
- •Assist with care coordination and communication among the care team
- •Maintain accurate and timely patient documentation
- •Ensure compliance with healthcare regulations and company policies

- Primarily caring for pediatric patients in residential and or school settings. Flexible scheduling opportunities, including full-time and part-time availability. Local travel required within the assigned service area.
