Occupational Therapist (OT) – Home Health

About This Role
As an Occupational therapist, you will provide skilled occupational therapy services in patients’ homes, conduct comprehensive evaluations, develop individualized treatment plans, and collaborate with physicians and our interdisciplinary care team. You will focus on helping patients achieve functional independence through ADL training, home safety assessments, adaptive equipment recommendations, and cognitive and fine motor rehabilitation. In this role, you will support patients across Souderton, Lansdale, Telford, Perkasie, and Quakertown areas.
Key Responsibilities
- •Evaluate patient’s functional status (muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, work capacity, etc.).
- •Evaluate home environment for hazards or barriers to more independent living, identify equipment needs and participate in the development of the total plan of care.
- •For patients who plan to return to work, perform work capacity evaluation and may refer to specialized vocational training in accordance with Organization policy.
- •Develop treatment programs and establish goals for improved function.
- •Communicate plan of care to referring physician and other Organization professionals.
- •May teach new skills or retrain patients in once familiar daily activities that have been lost due to illness or injury, in accordance with Organization policy.
- •May fabricate splints and instruct patients in the use of various types of adaptive equipment to improve function.
- •May train patients in the use of prosthetic and/or orthotic devices.
- •Maintain appropriate clinical records, clinical notes, and reports to the physician about any changes in the patient’s condition. Submits these records in accordance with Organization policy.
- •Maintain contact/communication with other personnel involved in the patient’s care to promote coordinated, efficient care.
