We are seeking dedicated, licensed Physical Therapists to provide home health services in Pompano, Fort Lauderdale and Sunrise. In this role, you will deliver one-on-one care, assess patients’ therapeutic and functional status, develop and direct individualized treatment plans, and educate patients and caregivers on safe, effective care. Key Responsibilities

  • Assess and evaluate therapeutic/rehabilitative/functional status and participate in the development of the total plan of care. Evaluate the home environment.
  • Assesses for muscle strength, mobility, gait, ROM—potential for rehab.
  • Direct physical therapy treatment.
  • Instruct patients and families/caregivers in the use and care of therapeutic appliances.
  • Determine priority needs for physical therapy
  • Report to physicians on patients’ reaction to treatment or changes in condition.
  • May train patients in the use of prosthetic devices.
  • Identify patient and family/caregiver needs for other home health services and referrals as necessary.
  • Prepare and submit clinical and progress summaries based on the attainment of goals.
  • Participate in discharge planning for patients.
  • Prepare and submit a clinical progress summary based on the attainment of goals directed by Organization policy.
  • Provide physical therapy consultation to home families/caregivers when indicated.
  • Provide service education programs for nursing organization personnel as needed.
  • Participate in the peer consultation process.
  • Ensure that arrangements for equipment and other necessary items and services are available.
