Key Responsibilities

• •Provide one-on-one skilled nursing care in the home setting
• •Deliver treatments and nursing services as directed by the care plan
• •Monitor patient conditions and report changes to the supervising Registered Nurse
• •Support implementation of individualized patient care plans
• •Educate patients and families regarding ongoing care needs and treatment plans
• •Assist with care coordination and communication among the care team
• •Maintain accurate and timely patient documentation
• •Ensure compliance with healthcare regulations and company policies

Qualifications

• Current Registered Nurse (RN) license in the state of PennsylvaniaRequired
• Minimum of 1 year of pediatric shift care nursing experienceRequired
