Depression and anxiety screening

Understanding Depression and anxiety screening
Depression and anxiety screening is a routine part of medical social work in home health care. Untreated depression affects up to 25 percent of home health patients, yet it is frequently overlooked because symptoms overlap with medical illness — fatigue, poor appetite, difficulty concentrating, and social withdrawal can all be attributed to the underlying health condition rather than a treatable mood disorder.
Screening uses validated assessment tools to identify symptoms early, when intervention is most effective. Medical social workers conduct these screenings in a conversational, non-judgmental manner, helping patients understand that depression and anxiety are medical conditions — not character flaws — and that effective treatments are available even while managing other health challenges.
How Screening Works
Your social worker will use standardized questionnaires such as the PHQ-9 for depression and the GAD-7 for anxiety. These brief tools ask about your mood, energy, sleep, appetite, concentration, and feelings about yourself over the past two weeks. Scores help determine whether symptoms meet clinical thresholds and guide recommendations for treatment.
What Happens After Screening
If screening indicates depression or anxiety, your social worker will discuss options including counseling, referral to a mental health specialist, and coordination with your physician regarding medication. Supportive interventions such as behavioral activation, structured daily routines, and social engagement strategies may also be recommended. Follow-up screenings track your progress over time.
When to Contact Your Care Team
Contact your social worker or physician if you notice worsening sadness, loss of interest in activities you previously enjoyed, persistent feelings of worthlessness, or difficulty functioning in daily life. If you or someone you know is experiencing thoughts of self-harm or suicide, call the 988 Suicide & Crisis Lifeline (call or text 988) immediately, or call 911.
