About This Role
CarePine Home Health is seeking a licensed Medical Social Worker to join our Greater Philadelphia team based out of our Horsham office. In this part-time role, you will provide psychosocial assessments, counseling, and community resource coordination for home health patients and their families.
You will work collaboratively with nurses, therapists, and physicians to address the social, emotional, and financial barriers that affect patient health outcomes. Your role is vital in helping patients navigate transitions of care, access community services, and maintain dignity and quality of life at home.
