Medical Social Worker (MSW) — Home Health

About This Role

Key Responsibilities

• •Conduct psychosocial assessments and develop social work care plans for home health patients
• •Identify and coordinate community resources, entitlement programs, and support services
• •Provide individual and family counseling to address social and emotional barriers to health
• •Facilitate transitions of care and discharge planning in partnership with the nursing team
• •Participate in interdisciplinary care team conferences and document all social work services

Skills & Competencies

• •Knowledge of Medicare/Medicaid, community resources, and discharge planning processes
• •Psychosocial assessment and crisis intervention
• •Interdisciplinary team collaboration and care coordination
• •Electronic health record (EHR) documentation

Qualifications

• Master's degree in Social Work (MSW) from an accredited programRequired
• Current LSW or LCSW license in the state of PennsylvaniaRequired
• Knowledge of community resources, Medicare/Medicaid, and discharge planningRequired
• Strong interpersonal, assessment, and documentation skillsRequired

• Minimum 1 year of healthcare or home health social work experience preferred
