Registered Nurse (RN) — Home Health

About This Role

Key Responsibilities

• •Perform comprehensive skilled nursing assessments and OASIS evaluations in the home setting
• •Develop, implement, and update individualized care plans in collaboration with the physician and care team
• •Administer medications, wound care, IV therapy, and other skilled nursing interventions
• •Educate patients and families on disease management, medication compliance, and wellness
• •Coordinate care with physicians, therapists, and home health aides to ensure continuity

Skills & Competencies

• •Wound care, IV therapy, and medication management
• •Electronic health record (EHR) documentation
• •Patient and family education and motivational interviewing
• •OASIS assessment and home health documentationPreferred

Qualifications

• Current Registered Nurse (RN) license in the state of FloridaRequired
• Graduate of an accredited nursing program (BSN preferred)Required
• Minimum 1 year of acute care or home health nursing experienceRequired
• Proficient in clinical documentation and electronic health recordsRequired
