Experience the difference in one-on-one home health nursing, where you have the flexibility to manage your schedule, build meaningful patient relationships, and truly see the impact of your care.  As a Registered Nurse (RN) with CarePine Home Health, you will provide skilled nursing care to patients in their homes. You will conduct comprehensive patient assessments, develop individualized care plans tailored to each patient’s needs. You will collaborate closely with physicians and an interdisciplinary team to ensure coordinated, high-quality care. In this role, you will support patients across the Reading, Fleetwood, and Hamburg areas.

Key Responsibilities

  • Conduct comprehensive initial assessments to determine individualized home care needs.
  • Develop, implement, and continuously evolve patient-centered care plans.
  • Perform ongoing nursing assessments and adjust care plans in real time based on changing conditions.
  • Administer treatments, medications, and interventions as prescribed—bringing hospital-quality care to the home.
  • Empower patients and families through health education and hands-on guidance.
  • Support proactive discharge planning to ensure seamless care transitions.
  • Serve as Case Manager when designated, coordinating holistic care and ensuring continuity.
  • Maintain clear, timely, and accurate clinical documentation.
  • Collaborate closely with physicians, care coordinators, and interdisciplinary teams.
  • Serve as a vital liaison between the patient, family, and healthcare providers.
  • Participate in case conferences and advocate for optimal patient outcomes
  • Coordinate necessary equipment, services, and resources for effective home treatment
  • Supervise and evaluate home health aides every 14 days, ensuring top-tier care delivery.
