About This Role

Key Responsibilities

• •Conduct comprehensive PT evaluations and develop individualized treatment plans in the home setting
• •Administer skilled physical therapy interventions for post-surgical, neurological, and orthopedic conditions
• •Collaborate with physicians, nurses, and the interdisciplinary care team to coordinate patient care
• •Educate patients and caregivers on home exercise programs and fall prevention strategies
• •Complete accurate clinical documentation and OASIS assessments within required timelines

Qualifications

• Current Physical Therapist license in the state of PennsylvaniaRequired
• Graduate of an accredited Physical Therapy programRequired
• Valid driver's license and reliable transportationRequired
• CPR/BLS certificationRequired
• Strong clinical assessment, documentation, and communication skillsRequired

• Minimum 1 year of clinical experience preferred; home health experience a plus
