Occupational Therapist (OT) — Home Health

About This Role

Key Responsibilities

• •Conduct occupational therapy evaluations and develop individualized treatment plans in the home setting
• •Provide skilled OT interventions for ADL training, fine motor rehabilitation, and cognitive retraining
• •Perform home safety assessments and recommend adaptive equipment modifications
• •Educate patients and caregivers on compensatory techniques and energy conservation strategies
• •Collaborate with nursing, PT, and physician teams and complete accurate clinical documentation

Skills & Competencies

• •ADL training, home safety assessment, and adaptive equipment recommendation
• •Fine motor and cognitive rehabilitation techniques
• •Strong patient and caregiver education skills
• •Electronic health record (EHR) documentation and OASIS assessmentPreferred

Qualifications

• Current Occupational Therapist license in the state of PennsylvaniaRequired
• Graduate of an accredited Occupational Therapy programRequired
• Strong knowledge of ADL training, home safety, and adaptive equipmentRequired
• Excellent clinical documentation and communication skillsRequired
• Current Occupational Therapist license in the state of PennsylvaniaRequired
• Graduate of an accredited Occupational Therapy programRequired
• Valid driver's license and reliable transportationRequired
• CPR/BLS certificationRequired

• Minimum 1 year of clinical experience; home health experience preferred
• Minimum 1 year of clinical experience; home health experience preferred
