Once completed, they will ensure documents are properly witnessed or notarized as required, and that copies are distributed to your physician, hospital, healthcare proxy, and family members.
When to Contact Your Care Team
Contact your social worker if you want to create or update advance directives, if you have questions about documents you completed previously, or if your health condition or personal wishes have changed. Directives can be revised at any time while you have decision-making capacity. If you experience a medical emergency, call 911 — inform responders if you have an advance directive or POLST form in your home.
