Transportation to medical appointments is a critical need for home health patients, many of whom can no longer drive and lack reliable alternatives. Transportation Options Available to You
Your social worker will assess your transportation needs and identify options based on your insurance, location, and physical abilities. These may include non-emergency medical transportation covered by Medicaid or Medicare Advantage plans, local Area Agency on Aging programs, veteran transportation services, community volunteer driver networks, and rideshare services with medical transport features.
Scheduling and Coordination
Many medical transportation services require advance scheduling. Your social worker will help you understand booking requirements, eligibility documentation, and how to request wheelchair-accessible or stretcher transport if needed. They can also coordinate transportation timing with appointment schedules to minimize wait times and ensure timely arrivals.
When to Contact Your Care Team
Contact your social worker if you have an upcoming appointment and no way to get there, if your current transportation arrangement is no longer working, or if your mobility has changed and you need a different type of vehicle. Plan ahead whenever possible — most services need 48 to 72 hours notice. If you experience a medical emergency at any time, call 911 for emergency transport.
