Safety and vulnerability screening is a confidential assessment conducted by medical social workers to identify patients who may be at risk of abuse, neglect, exploitation, or self-neglect. Home health patients are among the most vulnerable populations for mistreatment due to their dependence on others for care, potential cognitive impairment, social isolation, and limited ability to seek help independently.
Screening is conducted with sensitivity and without judgment. Social workers understand that abuse and neglect occur across all socioeconomic backgrounds and family structures, and that victims often feel ashamed, afraid, or protective of the person causing harm. The goal of screening is not to assign blame but to ensure the patient is safe and to connect them with protective resources when needed.
What Safety Screening Involves
Your social worker will assess your living environment, relationships, access to basic needs, and overall wellbeing during routine visits. Screening includes observation of physical indicators, private conversations about how you are treated, assessment of financial management, and evaluation of whether your care needs are being met. All assessments are conducted confidentially in a safe, private setting.
Protective Resources and Support
If concerns are identified, your social worker will discuss your options and connect you with appropriate resources. These may include Adult Protective Services, domestic violence hotlines and shelters, legal aid, guardianship or conservatorship services, and alternative care arrangements. Your social worker will develop a safety plan with you and advocate for your protection while respecting your right to make your own decisions to the extent possible.
When to Contact Your Care Team
Contact your social worker, nurse, or any member of your care team if you feel unsafe, if someone is hurting you, withholding food or medication, taking your money, or threatening you. You can speak confidentially. If you are in immediate danger — being physically harmed, threatened with a weapon, or denied essential care — call 911 immediately.
